Pathways Home Health, Hospice, and Palliative Care

Founded in 1977 by a group of Stanford physicians and community members, Pathways is a Bay Area pioneer, leader and innovator in Hospice, Home Health, and Palliative Care. As one of the nation’s very first hospice providers, Pathways has decades of experience helping people live their remaining days to the fullest, in comfort and dignity. Pathways provides home care, hospice, palliative care, bereavement support, and veterans resources. Care can be provided at home or in settings such as assisted living, a nursing home, or the hospital.
